So, a dermatologist has an MD, having gone through four years of medical school.
Then the dermatologist-in-training usually does one year of internship, which s/he spends in a hospital taking care of really sick people.
Then this person goes through another 3-4 years of training in dermatology, or of diseases of the skin, nails, and hair. To become a full-fledged dermatologist, this person then needs to be board-certified by the American Board of Dermatology, which sets the standards for the practice of dermatology in the US.
Dermatology is a very competitive field of medicine, so often times the dermatologist has done a lot of research in the field, and may even have a research doctorate, or PhD, in dermatology or related fields as well.
