When an outfield player in a Cricket match catches the ball and chucks it in the air. Has there ever been an occasion where the ball has come right back down and hit his head.

never seen that happen in international cricket, but im sure stuff like that happens in club cricket, but cricketers in general are real men, and can take the pain and not complain , i mean lets face it they have to be
Barehanded catching
Batters getting hit on their heads, chest and elbows.. there is a reason they started wearing helmets lol

Not that often in the outfield but quite common in the slips where there's more players around the ball.

I remember a fielder return the ball from the deep, and hit the sqiuare leg umpire on the back of the head.

I have seen it happen when the keeper tosses the ball away to the slips and the slip fieler wasn't looking...it hit him right on his temple...
