It could be she gave birth and ate the babies. Possibly she has a tumor and no babies;or some other inflamation that is making her sick. It is unusual for the area to be square. It's also very possible that her swim bladder is injured,and nothing will really help to heal her.

My best guess is that it has already had the babies or is in the process,
feamale guppies will eat their babies so she may have eaten them already or if they are in with other fish they may have.
if it is still bigger than normal she may be still in the process but sinking to the bottom and staying still is a sign of birth.
If in doubt go to your local aquarium they should be able to tell you whats wrong GOOD LUCK!

she probably flayed the eggs already. If you don't see baby guppies she provably ate them and one was unhealthy and made her sick
