The material. The fake is usually a much cheaper material. The real Boss is tightly woven and doesn't have a shine to it. The lining of the fake is probably polyester, the real is either silk or acetate. The fakes are difficult to spot because the counterfeiters pirate the labels. The stitching is almost always loosely done on the fake and the thread is polyester not cotton.The fit is not as good- the arm holes tend to pull and pucker.

Most of the time you can tell pretty easily but some boob jobs are very very good and it is hard to tell. Also, if the girl had med/large boobs to begin with it makes it more difficult.
