It could be one of two things, either constipation or a swim bladder infection. For constipation fast him for 2 days and then try to feed him a fiber rish foor like a pea. For a swim bladder infection you need to treat with Furanace and medicated foods for at least 10 days.

Swim bladder is often caused by constipation. I had a fish on his side for about a week in this situation.
Some people suggest feeding the fish a pea which will help get rid of the constipation but getting a fish to eat a pea is quite difficult.

An easier way is to add 1 teaspoon of Epsom Salts to 40 gallons of water. This also acts as a laxative.

Don't remove your fish from this water until it is swimming properly again.

I've saved my fish from swim bladder problems this way.

at walmart they have different things that you can add to the water for certain conditions in the fish. if you read the packages they tell you what to look for and just pick up the one you need, or do what I did, get them all. You probably end up using all of them at one point or another.

YOU HAVE A FUNGUS PROBLEM.
treat for that--remove carbon from filter.
replace 7 days after signs of disease are gone.
it will take medication out of the water.
