Gene mutation is when nuclecic bases are deleted or changed and chromesomal mutation is when the chromesomes don't separate correctly or they don't form. This is usually during mitosis or meiosis.

X-rays, radiation etc.
The reason why we have to wear those clothes before taken an x-ray is to prevent mutation if you notice it covers the chest in order to protect the heart. it also covers the reproductive organ to protect gene mutation.
