when a scholarship states "list any activities... "?

should I make a list or should I write them in a sentence.
ex.
basketball
Golf
tennis
or For the past tow year I participated in ... and then I played ...

A list rather than a narrative would be more appropriate.

However, you will want to be much more specific than just "basketball."

Your list should look more like this:

3 years junior varsity basketball, team co-captain
1 year varsity golf team, semifinalist in regional competition
2 years tennis, Greenville Country Club recreational league
