It's a 'wise guy' term coined during the time of Italian crime families.

Most folks nowadays just call it a 'tune up'...an attitude adjustment done with fists and feet...perhaps a blunt object.

Thats when Guido and his associates tell you what the problem is and then proceed to beat it out of you.If this tune-up does no good then the next one will be the last tune-up you ever get.

To keep this simple and actually answer your question. Back in the day of FIATs (and even later with other european cars, VW digifant systems etc) people would run into problems with the engine. The problems arose from the car being driven short distances with little throttle (and low RPMs). Eventually carbon buildup would cause issues. The cure is to basically warm up your engine and run it to the limits of each gear, in every gear, for a good while.
