It means his testosterone level is lower than the average male's. There are many possible causes for this condition and he may or may not have symptoms as a result.

He needs to get a lot more exercise. Testosterone is a natural male hormone. Your level of activity and care for your body has much to do with the level of hormones your body will produce.
