I am trying to correct a situation where a member did not completely fillout all 4 regional brackets. How can I help get this fixed for that member.

Set up the filing procedure so that it will not be accepted if any brackets have a missing or unfilled area. This is where on line filing is best at.
If there are no typed entries on particular fields, the application gets rejected.

The scenario your described screams out that the fault is in your ballpark.
Who received the application?
Why did it even get to the commissioners desk when it was not filled correctly?
Go back to the rule book. If it means they are disqualified because of this, then they are disqualified.
Soemtimes it needs to be reinforced. Sure they will not play this year.
If that sounds too unfair, gather the people who are stakeholders and have a meeting.. Determine by consensus what to amend on the rules, or create a bylaw.

Whatever you do, Do not fix this by yourself.

after the first game started, all the breackets have been locked, there is no way to fill in the rest of the bracket.
It's just that person's loss, there is nothing you can do about it. If you let them fill out a paper bracket and give it to you, they would have an unfair advantage

they should get the games wrong for being stupid and not answering, it gives you a better chance to win anyways...if its for money even better
