Everybody "lives in their head" a certain amount each day. It's called focusing on your thoughts.
But some people focus on their thoughts and feelings a lot more than they do the world around them. They become less active, more reflective. This can become a real strength. Wisdom comes from relfecting about life. Intellectuals focus on their thinking a great deal, analyzing and learning more about their favorite topics.
The opposite of someone who "lives in her head" is someone who never has an unuttered thought. Or people who do things without thinking about the consequences. Or people who are involved in so many things they can't follow through with most of them. Or people who want to do everything, so they can't decide what they want to do.

People like this are sheltered. They usually have odd child hoods and are quite intellectual. They often have OCD or nervous twitches, and they talk to themselves alot. They dont like things to get out of hand, and they feel the need to be in control of everything. If something changes they have a breakdown usually. They aren't very socialy skilled.

My first wife had two college degrees, was a member of Mensa, but gradually constructed a reality for herself based on Tolkein, New Age philosophy, and Anne Rice novels. Eventually, there was no room in this world for family, old friends, or people who genuinely cared for her personal welfare, only hangers-on and would-be predators.
I'm a cartoonist, so I guess I sometimes dwell in in a separate realm myself, but I treat it as a job and leave it in the studio in its set hours.

One answer is: "intellectuals", but the truth is, we all live in our heads: we have no way of disproving that we are not merely brains in tanks, being fed impulses from computers, with the possible exception of suicide, which seems a little extreme to prove a point.

Basically, these people believe the world revolves around them. They are oblivious to having common sense about the world, their lives and of those around them. Almost like a version of a soap opera . Always juicy but has no substance.
