Well, it depends on what the doctor is doing at the time. If they are looking at your chest, the doctor is looking at how your chest expands and contracts, making sure that it moves the same way on both sides.
If they are listening to your chest, the doctor is determining if your lungs are clear, or can be cleared by simple coughing. If it can be cleared with coughing, then what ever is in there, isn't as serious as a few other conditions.
Doctors also make you cough during a hernia check. If they have your genitals in hand, or are simply observing the way the genitals move during coughing, the doctor is seeing if a hernia is present.
In men, some hernias can be felt in the scrotum, and when a man coughs his testicles ascend and descend. A hernia wouldn't. Thus the basis of the "turn your head and cough" test.
There is a method to the medical madness, but I agree sometimes it is a little odd, and confusing.

This test is most often used with males. The doctor will feel the abdominal wall above the testes, a weak point in the abdominal wall.
