I think you just have to sit them down and just tell them. Explain to them why you kept it a secret for so long. I would approach them like this..." you want me to be happy right?" of course they're going to say yes bcs they love you. Tell them that you are gay and that you have a girlfriend and that you are happy. You know them, so you probably have a pretty good idea as to how they will react, just try to anticipate what they are going to say and have answers for them. But no matter what they say, it's your life and you have to be true to yourself. Remember that you're not doing anything wrong and this shouldn't be a confession.

I take it from the picture you are a girl too?!! I don't know how old you are and if you live on your own. That would help, however I have had gay friends, male and female. It is really sad that some family members don't support you in this situation. That is what is so scary about telling. I have always told my children that there is nothing that they could ever do to make me not love them and I am always there for you. My advise is to just tell them. Leave your girlfriend at home. See how they take it. Be sure you are able to handle there rejection JUST IN CASE. Your relationship, hopefully will last forever. If you mom and grandma are unable to accept this news, who are you going to live your life for? YOU or THEM

Just tell them, I am sure that they will love you no matter what your choices are in life.
I am a happily married heterosexual woman, but I have learned that life is too short to live in fear and live for other people and their idea of what your life should be.
I have lots of friends and family members that have chosen a different lifestyle than my own, that does not make me love or value them any less.
When most people choose to "come out" to friends and family, there may be some initial shock, but most people will tell you that they always knew.
Just be honest, and I wish you and your girlfriend the best. God bless you both and your families.

just tell them as i did all my family and friends... just start out with something like," by the way , i'm not like most people, I find the same sex attractive and well i am pursing these feelings. I still love both of you and i hope you still love me. " then eventually bring your gf around a little at a time, and eventually they should come around. hope it works and goes well as my "coming out" went. of course i think they had already suspected, being i never really dated, or hung around opposite sex....

I would just tell her and maybe say that you just got a girlfriend that youve just been friends with for a while.

Just mention in a light conversation one day,Like your talking to your friends about her. For example: " Oh...yeah me and my gilfriend ate there last week, it was great!" maybe you should go there for lunch some time." " By the way...is American Idol on tonight?" LOL Maybe that would work!!!!

You're a girl, right? They might be hurt - picturing the ideal husband/wife wedding for you - but it is your life, and if they love you (which they should!), then they'll accept it.

why do you feel the need now to tell them? it's your business and your choice, but, i just wondered why you want to tell them now all of a sudden. they may already suspect. did they ask you about her or something?
