Say "Thank you very much for including me. Dinner was lovely." A thank you note later is always nice to receive. A hostess gift is not necessary. Your date should provide that.

Thank you for a lovely dinner, I had such a wonderful time and I hope we have chance to do this again sometime at the end of the night -- bring a hostess gift of course. No need for notes unless it was a really formal dinner.

A "thank you" helps, but asking beforehand if you can help in any way is great. Also, bringing a small gift is nice (like an elegant desert or a box of chocolates or a fine wine).
Good luck!!!

As you leave thank him/her for the wonderful evening and then say how happy you were to meet him/her or see him/her again if you've met him/her before.

Your date and you might send a thank you note with flowers or some chocolates as well a few days after the party again expressing your appreciation for his/her hospitality.

Upon completion of the meal, just say thank you that was a lovely dinner.

Upon leaving, "thank you for a lovely evening". "Hope to see you again sometime" (only if you want to see them again.)

No card is necessary.
