I often misinterpret what my man says to me and lash out and say things that are hurtful for no reason at all? What is wrong with me - is it ADD?

Maybe you should ask him to break it down to you. You should think before you speak. Did something happen to you in the pass? relationships? family/friends? or teachers? Did they say something to you where you felt defenseless? It prob became a habit where you felt like a victim, and now you are lashing out to protect yourself whether you need to or not. You just need to relax and take it easy.

Not necessarily. There could be a number of reasons for this. You might just need help learning good communication skills. Try taking a deep breath and counting to 10 before you say anything. This gives you time to think about what was said and ask what the meaning was.

Another possibility is you are not misinterpreting what he says and he denies your correct interpretation
I'm not saying that is the case but you should consider it.
If you have this problem with many people then you would benefit from counseling

Maybe you have anger problems, and you can't find anyone to put it on except him?
