I might ask their opinion of the design or placement (not if I should or should-not get one). Their disapproval of the idea wouldn't affect me going forward with it....just as I wouldn't try to tell them what they should or shouldn't do with their body, even though if asked, I will express my opinion.

No. When I got my second one I didn't bother to ask my fiance at the time even though I knew he didn't care for them. Just because he chooses not to get any doesn't mean he can dictate what I put on my body.

I didn't ask because it had nothing to do with the person I was dating.
I don't think you need someone elses' permission when it's your body.
