Personally, in my experience, it has been a combination of the psychiatrist and me. The doctor has the expertise and has suggested medications, but sometimes he would give me a choice based on the side effects, etc. I would chose one or the other and try it out.

In other cases, the doctor clearly knows what I needed. For example, if I was manic and could not make clear decisions, he had to make the decision.

In most cases, you can always talk to your doctor if you disagree. They can't FORCE the medication down your throat, so you have the freedom to chose.

The doctor. Not everybody knows the uses of the meds, or the doses, or the time they need to be taken. If everybody knew, everybody would be able to treat themselves and doctors would be unnecessary.

You wanted Prozac and the doctor did not want to give it to you, was it you? As I answered in the other question, you have to tell the doctor your reasons to wish for a treatment, and listen to him or her back.

Read about fluoxetin in a vademecum, do a research, ask the doctor and find out what happens with that medicine.

my doctor and my psychiatrist work with me and if i disagree
then they don't put me on those meds
and beside sometimes i choose the meds i take
because i do research on them and then tell my doctor about and then she does her job
