No. Everyone lets their mind wander occassionally. If you see a really good looking girl/guy on the sidewalk, you'll probably think certain things at that moment. You may even become infatuated with someone else. However, being faithful is determined by your actions, not your thoughts. As long as you deny these urges and remain loyal to your partner, you are completely faithful... maybe even more so than someone who has no sexual thoughts about another person, because it's much more difficult to resist urges than to remain loyal without any urges to wander. This is a test of your commitment, and if you pass it, you are a good person in that respect.

Well, yeah, I feel a little guilty, but I know in my head that it's totally normal. I'm just a furless monkey, and I'm a good little monkey as long as I don't actually act on these fantasies.
If you care about your current lover, beware of persistent fantasies about one specific person. This might be the kind of thing that destroys a relationship, if there's something behind it.
PS: To feel less guilty, fantasize about someone you don't know and will probably never meet, instead of someone you see all of the time.

No, but you'll do well to keep those 'frequent fantasies' to yourself, unless of course you're talking about someone clearly unattainable. For example, I shouldn't think it unreasonable to fantasize about movie stars. But you get into trouble fast when you start fantasizing about the pool boy or the girl across the street - and you tell your partner.

Absolutely. You are lusting for someone else and that is wrong. If you are married adultery is not only with the body it is with the mind as well. If you are just dating then I think this means that you don't want your partner anymore and you need to fiqure out what is wrong.
