No, your heart is an organ in your body that pumps blood. When you die it will rot just like the rest of your body. Your consciousness is maintained in your brain, when you die and your heart stops supplying your brain with oxygen the brain dies, consciousness ceases, and you no longer exist. The brain will rot along with the rest of your body. Nothing goes on forever. 5 billion years in the future the Sun will grow hotter, become a red giant star and vaporize the Earth. Have a nice day.
