You should contact the Personnel Office of your last duty station and ask them for the status. They may have forwarded it, but may not have the correct address. If you were discharged from a separation activity, like Personnel Support Detachment, you may want to contact them as well.

Keep them informed of your address, if any changes have or will be made since your discharge.

I believe that in theory you are still able to log in to the MyPay website to retrieve that document. If that does not work you may have to go to your nearest military installation and talk to someone there about the issue.

Call the payroll office of the base you were discharged from. They will be able to help you.
