Registrar of Birth & Deaths will only give a certificate having Mothers' & Fathers' name, time& date and place of Birth. That is not a Birth Certificate. Birth Certificate is the one which is recorded and given by your school. The school leaving certificate, is the Birth Certificate, which has your Birth date & name.
If you want change that, then it has to be changed in the gazette. For that contact a lawyer, who will guide you.

The document issued by the Registrar of Births and Deaths is not the Birth Certificate, it is an extract of the Birth Register maintained by him under Registration of Births & Deaths Act, 1969. Get inserted your name therein (if necessary with an affidavit) and obtain another Birth Register Extract, mentioning your name.
