he will not let me go near the whole area whilst changing him, or in the bath when trying to clean his penis. is he just sensitive, or is it something more?

UTI's are rare but not unheard of in boys. Does he have a fever? If not, it probably is not a UTI. His penis might just be a little irritated from being wet too long. My two boys have both had this irritation problem before. What I've done is let them walk around naked for a while to give it some air and time to become dry. After a couple hours, the irritation goes away.

It would be a good idea to take him to his doctor to find out. It could be that his penis is just irriated from being wet or maybe he possibly hurt himself some how. Talk to him about it and ask him where it hurts and if he knows where or how it got hurt. It might also be helpful if you ask him to show you where it hurts but tell him that you won't touch it if he doesn't want to that way when you go to the doctor you will be able to have some sort of idea to tell the doctor where or how he might have hurt it.

If he is intact he could be partially retracting himself which causes new and odd sensations on his sensitive glan. That's normal. Let HIM retract himself and do not ever retract his foreskin or let a doctor.
If he's circumcised...no clue.

Bring him to the pediatrician just in case. It wouldn't hurt to see the Dr.

I would suggest talking to him about it. Ask where it hurts, or if he knows how it got hurt, or why it hurts. Then, take him to the doctor.
