How do you say middle name in spanish?

The thing is we don't use the names the same way than Americans do.
Usually, after the Christian name comes the father's name, and the mother's name goes last. In some countries, like Argentina, they use only one name (the father's).

So if by middle name you mean your mother's maiden name, then it is: apellido materno.
What for you is the last name (father's name) is: apellido paterno.
The first name or Christian name is: nombre de pila.

If the middle name for you is a second Christian name, then it would be: segundo nombre
(many people have 2 Christian names in some Spanish-speaking countries).

E.g.:
Juan Alberto Pérez González
Juan Alberto = nombre de pila
Juan = primer nombre
Alberto = segundo nombre
Pérez González = apellidos
Pérez = apellido paterno
González = apellido materno
(The main name is Pérez. You could call him just Juan Pérez).
