No. Forgiving your brother is your conscious decision that you will not hold that act against him; it does not depend on anything he does or on his "deserving"it.
Why? Because this is how God forgives us. "...God demonstrates his own love for us in this: While we were still sinners, Christ died for us."

There is a difference between forgiveness and reconciliation. It only takes one person to forgive (YOU). But reconciliation takes both people....one asking for forgiveness and the other forgiving. Forgiving some one doesn't mean that you have to put yourself in a position of being hurt by that person over and over again. Forgiving a person and trusting a person are two very different things.

No. Him repenting is between him and God, not you. Human beings are suppose to forgive even if forgiveness isn't asked of them. Repenting is a way of telling God (and God alone) that you're sorry for your sins.

no, spiritually it would b a greater accomplishment for you to forgive him even if he didn't repent. That is the kind of christianity christ loves best. But that doesn't mean for you to let him use you as a door mat.

No.
It is our duty forgive all people. Whether they repent is their own business. If we don't forgive the person who has sinned we have sinned worse than they did.

No, you should forgive him regardless whether or not he/she wishes to be forgiven (for your own sake) However, I don't think the person is forgiven by God unless they repent. That is my understanding of what the Bible says.

It is your choice to forgive him, regardless of whether or not he repents.

No, it would be nice for his to apologize and ask you to forgive him, but he doesn't have to. It will probably make you feel better if you forgive him anyway.

No, you can forgive him anytime you want to. It's best to do it as soon as you can because you never know what might happen in the future.
