Let me just get this right, you want to go from Hong Kong International Airport to Macau International Airport directly? (or by the most direct route possible).
The answer depends on what you are 'doing'. If you have just flown into Hong Kong then what you do is that you DO NOT go through immigration INTO Hong Kong. Instead catch the Airport Express Ferry direct to Macau. The ferry leaves from Hong Kong Airport. It's about 35 minutes to Macau. It will take you to the Macau Ferry terminal. You are actually processed through immigration in Macau. From there it as a 10 minute taxi ride to the the Macau airport. Check with the airport (HK) or your travel agent before you go, though, as there have been a few 'interruptions' to the service already.
If you go THROUGH immigration INTO Hong Kong you can not catch this ferry (you are not allowed to). In that case you have to take the train or bus into Hong Kong and catch the Hong Kong to Macau ferry. You will end up taking twice as long to do this than if you had caught the ferry from the Airport. The train ride into town takes about 20 minutes, and the ferry takes 55 minutes, but add in time for walking etc and it adds up. Then of course you have to take a taxi (or the slower bus) from the Macau Ferry terminal to the Macau airport.
There is a helicopter link between Hong Kong Airport and Macau but don't even think about it unless you have (many) hundreds of dollars (US) to burn. There are very, very few plane links between HK and Macau (and no discount ones), basically it's a deal to keep the ferry in business.
You can take the train or a bus between HK and Macau, which gives you a chance to see a bit of the country side. It's a 2.5 hour journey by bus (the better option). Note that you'd actually be taking the bus from Hong Kong to Zhuhai, which is the Chinese town that is just across the border from Macau. A new bridge (2008?) will make the land based route much quicker. From the Macau border it is a 10 minute taxi ride down to the airport.
You have put your finger on one of the more bizarre intercity transfer problems, congratulations.

grab a ferry, it's pretty cheap and will take about an hour or so. when you get off of the ferry, catch a public bus to the airport. it's fairly simple, i had to take this route with some friends last year. it seems to be a bit cheaper to fly out of macau than it is to fly out of hong kong.

I would suggest to take a ferry.you can take a ferry from Star Ferry Terminal in Kowloon, Hong Kong. It will cost around USD 30 one way. The journey will take about 1 hr +. It is quite convenient.
By air is very expensive.

No flights from Macau to HK, so the only way from Macau to HK is by Turbojet. Macau airport to ferry pier approx 20mins, one way ferry (Macau-HK) is around HK$275.00 (Super class) then from HK ferry pier to HK airport, taking the airport bus is around HK$47, Airport Express (train) is HK$100 taxi is around HK$300. Alternatively, you can take the helicopter from Macau to HK, one-way costs HK1,900-00.

take a ferry. i always take turbojet. they go straight from hk central to macau and it takes you just one hour. dont lose all your money while visiting the casinos over there though ;)
