Ship traffic is watched by satellite. The ship movements are compared to shipping manifests and lists supplied by the various shipping companies.
If there is a ship out there not listed or not on course as it should be, it gets a visit BEFORE it gets to the US port by the Coast Guard.
If the ship is doing something not in keeping with its past pattern, it also gets a visit. For example, if the ship usually carries bananas from Peru to Los Angeles and is now on the way to New York City with a load of car parts, that is going to raise some eyebrows.
If the ship is listed by a new company with a questionable background, the ship will also get a visit.
Lastly, radiation detectors are very sensitive and if a ship approaches any major port and the detector even CLICKS, the ship gets a visit.
But with any system, it can be breached with enough effort.

They're not. Besides, terrorists don't have to sail a bomb into NY harbor or some big city harbor; any port or or even small harbor pier would do. The word is TERRORism. Any place would TERRORize people. The physical damage may not be as great, but the psychological impact would be enormous...

It's Called good Intelligence (Something that they sadly seem to lack these days) You see, A truly good intelligence network
is able to know who has and gets what and what they may
intend to do with it..from there the action develops and plans
are initiated and executed to defeat the attempt.
Now honestly, to me the system is too political.What they need to understand to be effective is that a bomb or terrorist
doesn't know or care about the difference between a Democrat or a Republican and get back to what Intelligence is supposed to be used for...to protect the people of the Country

It wouldn't be that hard. If we had information that a nuclear bomb was on way to NYC then they would intercept it before it got there.
Ah yes, the assumption, "if we had information" that's really the CIA's job and I assume that smuggling a nuclear bomb is something much easier to happen in fantasy. But it it happened, which is not unlikely, then NYC would be toast.

ive looked into the terrorist with a nuke in newyork thing its not really plausible because nuclear weapons need huge amounts of maintanance things that most governemnts cant afford let alone terrorist if they got ahold of a 20 year old russian nuke the charges will fire throwing the uranium around but there will be no fisson reaction no mushroom clud no hiroshima
