After sterilizing a needle in alcohol, you can slide it into the edge of the blood blister and drain it. It won't hurt much, unless you are careless enough to poke it clear through!

If you just pop it, you are opening it up to infection. So if you do, "pop it" then you need to clean in and disinfect the area and then treat it with antibiotics and cover it so it will not collect dirt and so forth. Change the bandages daily until new skin grows over the wound.

I have always pinched the top off with nail clippers. Works well for me.
