You need a prescription from your doctor stating that you need one. Then you go down to the DMV and they will give it to you.

It depends what state you are in, or country for that matter...In California, you only have to get your doctor to sign the DMV form that you can print from your home computer, and bring it into the DMV. You do not need to go to the Secretary of State, as one person said, because that is ridiculous: don't you think the Secretary of State has better things to do than take THOUSANDS of forms for handicap placards?

Your doctor has to fill out a form then you take it to the Secretary of State and they will give you a sticker
