When I received my insurance bill this time, there was a lien put
on the insurance information. The company claiming to be a lien holder is a company in Arizona that I have had no dealings with. I have a clear title to the car. The car has never been in Arizona, so it is impossible for it to be a mechanics lien. Anyone had similar problems?

If you have clear title and only your name is on it, then I wouldn't worry too much about it because that's all you need to sell it. Sounds more like an insurance company mistake and you should contact them about it.

You say you have the title to the car, contact the DMV, they will tell you if another title has been issued, and why. You would have received a letter from them stating a lien was going on the record, and to submit your current title as it would be invalid. It sounds like an error through your insurance company, call them and get it cleared, this could cause you serious issues.

You had better get that cleared up pronto. That could be a very costly complication. What you might have is a collection agency who has gone through the steps to put a lien or judgement against your property. They found your vehicle and attached that. It seems that would be hard to to without you getting any notices, but if they took it through the legal process, you might be on the hook.

I would contact them to get what information they have. After that, I would contact the DMV in your state to find out how this happened.

It's simple mistake by your insurance company, call them and have it taken off. Nobody has the right to be a third party on your insurance without your permisson. There's no need to contact the company in AZ, unless you get a letter from them requesting to be put back on the policy.

So find out what it is.

Possible they made a mistake, call them up and ask "Who are you and why did you put a lien on MY vehicle?"

Demand information out of them, don't give them any other than your basic ID info.
