I agree with the poster above me. With this little info, there are many things it could be, but some could be serious. A vet trip sounds like it is needed. Good luck.

Hi,
The best thing I can suggest is take him to the vet as soon as possible. Unfortunately, I had my 15 year old cat put down a week ago with a similar problem. He turned out to have a galloping heart murmur and hyperthyroidism which in turn caused renal failure. It wasn't very pretty. Make sure you do the best thing for the cat.
I wish you the very best, I know how heartbreaking it is.

See your vet immediately. Your cat may have internal parasites that are preventing the absorbtion of nutrients, which makes them lose weight. They can die very quickly from this and pass it on to other cats in the home via the litter box. Make sure your cat drinks water, and take a stool sample with you to the vet to request a parasite test.

Could be anything from cancer to an infection to feline AIDS to worms to a million things. You cat needs to go to a vet ASAP. Meaning call NOW to make an appointment within the next 24 hours.

Was the weight loss sudden? Is he eating and using the bathroom normally, as in often? Either way....
He sounds malnourished and dehydrated. These could very well be signs of anemia and kidney failure. Both are common and treatable, but you must take to vet right away!
