Sean I was a Navy recruiter for many years. The info is for your "dog tags" so a proper service could be held in the event of death. "None" or "No religious preference" are options to enter on the forms.

So that if you want you can be laying there in pain, swearing and have two officers (nurses) telling you "No no no... you should say 'praise Jesus'", no matter what religion you follow or none.

Seriously... because they're SUPPOSED to offer you the proper spiritual guidance if you need it, meet any dietary needs you have (what a joke, I hardly EVER saw this happen properly without having to choke the living daylights out of dietary over the issue and CHCS SUCKS!!!), and cultural sensitivities... last rites if you need them, burial issues.

(sigh) it all depended on the staff caring for you whether or not they were doing the job they're supposed to do as opposed to the job they actually did.

Um... do I sound BITTER? Grrr...

When it comes to medical forms, I suppose it's to alert them to possible conflicts involved with religious persuasion. For example, some Christian Scientists don't want certain medical procedures don't to them so I suppose it lets the medical professionals know what to watch out for.

For the military, I don't know. I suppose it could have something to do with worship privileges but... I'm not sure. My husband could probably tell you more, he was in the Air Force.

I know the military asks for burial reasons, should you die in battle. Or at least that's my husband's claim. I would assume it is the same for medical forms....just incase you die, they want to know if they need to call in a priest or whatever.

I'm sure the answer given by the questioner would tell you that it is so they will know how to treat you in case of death. That would also probably be the correct answer. I would be willing to bet that they also use the information to gather statistics for interested parties.

Because they want to know how to go about giving treatment to each and every individual person. Different religious beliefs don't believe in certain treatments.
