Yes, this has happened to me before. This happens if either the condom was improperly put on or if the man loses erection during the intercourse or comes inside and doesn't withdraw quicly enough. Makes sure that the woman gets tested for STDs and gets a morning-after-pill in that case. It's dangerous.

Yes. While having intercourse, the male can become somewhat "limp" again which will result in the condom falling off inside of her. Or the condom is just too big for the guy and will slip off.

yup, thats why you gotta make sure to squeeze the tip of the condom after you put it on so that there is no air in there making the condom loose. Make sure you roll it all the way down to the base of the penis. And when taking it out of the woman after you have "finished", be sure to grab hold of the ring of it near the bottom of your shaft, that way it doesnt slip off after your erection gets smaller from "finishing"

It can if your are not buying the right sized condoms. Stay away from the magnums man. If this is a girl he might not have been fully erect also you might want to look into lubrication so the friction does not build as much.

yes and lucky for me I was able to catch it just in time. But it can fall off and get stuck inside the vagina and you might have to go in and dig for it or have a doctor remove it. Yes if his sperm spills out you can get pregnant and worse yet get a STD.
