Absolutely nothing. I do not validate other peoples stupidity and you shouldn't either. People like that are simply not worth your time. All you do is give a look that says I think you are stupid and walk away. Saying anything back just allows the conversation to continue and seriously what is the point of that?

First of all I'd tell them that is not the political correct way of saying what they said. The term is developmentally delayed. And then I say "Just because I am beautiful it doesn't mean I don't have a good brain". "Maybe you should have yours checked out".
