Hey, I remember when I was confirmed yes they did asking me for booth baptism and communion certificate. If you don't have any of the documents you can tell the priest what church you took the sacraments in any he can contact the church. God Bless and may the Holy Spirit confirm you into the catholic church.

You either need the documents or know which church you were baptized in. The church will have a copy of the baptismal certificate and First Communion certificate.
Go talk to the people in the adult education program at your local church. Based on how faithful and knowledgeble a Catholic you are, you may not need any kind of faith formation before Confirmation.

You will need an original copy of your baptismal certificate. This should have the information about your First Communion on the back.
If you're a teen, talk to the youth minister at your parish. If you're an adult, talk to someone in RCIA or the pastor of the parish. You'll have to go to some classes, but it isn't a huge thing in most cases.

Contact the Director of Adult Formation in your local Parish. He or she will guide you in the right direction.
In general you will need to have documents showing Baptism and First Communion, but if you don't have them, the Parish will help you get those.

I think all you'd have to do is contact your nearest parish of choice, take them copies (or send) of your Baptismal records and First Holy Communion stuff... I'm sure some kind "soul" there will guide you.
Man was I glad I didn't have to do that as a Byzantine Catholic.
