You hyphenate them when they are used as a multi-word adjective. For example, if you just say, "We had a session for questions and answers", no hyphen is needed. However, if you say, "We had a question-and-answer session", the hyphens are needed--it is not a "question session" or an "and session" or an "answer session", so you use the hyphens to show that the modifier is in fact one compound adjective.
The same applies to one hour. In "It will take one hour," no hyphen is needed. However, "We will have a one-hour session," requires the hyphen for the same reason mentioned above.
