The Form 1040 has changed. File the Form 1040 for the year in question. If you are filing for 2004, use the 2004 Form 1040.

Prior year forms are available at irs.gov.

The form changes each year. You must use the 2004 form for 2004 tax returns, the 2005 version for 2005, etc.
