You can call the IRS at 1-800-829-1040 to report the person. You can also go to the irs website and download a Form 3949-A and fill it out and mail it to the IRS. It's called an Information Referral. Basically you are reporting a person for committing illegal tax activity. You do have to give them your name and address as well.

Just file your tax return. The IRS will be in touch with you to clear up the issue. It would be a good idea to pull your credit report and check for signs of identity theft.
I've had this happen twice. The IRS contacted me and it took a couple of days to sort it out. On one occasion the name used by the other party didn't match mine and the IRS dropped the case without further action. On the other occasion the other person had my name and SSN correctly. I had to provide a sworn statement that I had not worked for any other employers other than the ones reported on my return. They never got back with me after that so I assume that they tracked down the other party and handled it.

This happened by accident to my husband. The tax preparer filled out the return with the wrong name, but my husbands SS#. It took us a few months to resolve it, but there was no penalty to us. It is a matter of notifying the IRS and stating that your return is the only one that is valid, and that the other return is not yours and not your employement.

You will need to notify and file reports with several different agencies.
1)File a police report
2)File a report with the Federal Trade Commission (FTC)
3)File a report with Social Security
4)File a report with the IRS.
