first, calm down. second call the vet NOW!! and maybe someone can help you try to corral him. catch him and take him straight to the vet. you need all the help you can get sounds like, so get some friends to try to help you catch him, or the vets office may be able to catch him. but sometimes if their old, its better to not cause them so much stress, you might just let him die peacefully

I'm very sorry to hear your cat is sick, honey. You may want to consider if it is kinder to end his suffering and put him to sleep now, or if you want to drag it out knowing he'll only feel worse. Your vet can help you make this decision.
You do need to take him into the vet right this minute if he is in so much pain and so unwell. Grab a big blanket or towel and throw it over him, and then wrap him up like a little bundle with just his head poking out. Get a friend or family member to help you and take him to the vet. Do what's best for you cat, even if it is emotionally painful for you.
Google the "rainbow bridge" for some sites to help you deal with this difficult transistion.

Well, I'm befuddled. All these people telling you to take him to the vet, when that's what you're trying to do, but the problem is catching him first. I always used bribery, meat or cheese, but if he's that ill, he may be past being bribed. If he's to the point where he won't open his eyes, and his body is cold, then it shouldn't be too hard to grab him. Good luck.

Call your local animal control and let them know what is going on. They should come out with a humane trap or have some way to catch your cat.
Because he is sick food and tranquilizers will not work because he will not eat them.
Whatever you do don't chase him!! This will not accomplish catching him and will only stress him out and make him more sick. Unfortunately, when animals get sick they often crawl away to die alone. This is heartbreaking for those who love them, but it is instinct for them because they are vulnerable.
Call your vet and then call animal control!!!

Your cat is in a lot of pain if it cant breathe. You need to focus on your cats comfort. This may mean having it put to sleep and out of its misery. This is very hard to do, I did for my cat that was 12 years old. It sounds like your cat is anemic and that does cause suffering, its like having a constant asthma attack. Please take it to the vet! To catch it, try putting out some tuna or something that smells really strong.

it's a very sad situation. You have to try to catch him, get help doing so. While you doing that try not to scare him too much, it would be too stressful for him. Take him to the vet immediately and tell them it's an emergency as soon as you get in there. I hope he gets better, good luck and best wishes for the darling kitty.

Is he outdoors?
Have you ever been able to touch him?
Be patient, move slowly.
Consider renting a human animal trap from a humane society.
Provide him with a warm bed to sleep in, perhaps with a heating pad under it so that he will be tempted to go there, and when he is resting you can CAREFULLY pick him up and crate him to take him to the vet.
It sounds like he is in great pain and I hope you can catch him soon.

Im so sorry. The only thing you can do is get a trap and catch it to take it to the vet. If the vet says its hopeless just put it to sleep. You can borrow a trap from animal control. :(

It sounds like a metabolic problem cats present for lack of a certain aminoacid. Wrap him in a towel so you can transport him without getting clawed and have a vet see him.
