This is because contact lenses are medical devices and wearing them can affect the health of your eyes. The fit includes measurements for diameter and base curve. Prescriptions are required for all contact lenses to keep your eyes healthy.

I dont' know. Check with an optician about this. Do you have a prescription that needs to be included? If so, you need to have an optometrist or ophthalmologist measure your eye for the right prescription and the right diameter of the eye etc.

Not necessarily a prescription (unless you also need contacts for vision correction), but will at least need to go to the optometrist to be properly fitted for the contacts.

YES. I had to go back to my eye doctor and have her add colored contacts to my prescription paper before the pharmacy would give me colored ones.
