The following info may complete answer nr 2. When the colonial powers left their respective African colonies (in the sixties) the malaria had almost totally been eradicated by quinine therapy. The local new governments did absolutely nothing as far as water management was concerned, stagnant waterpools reappeared, excellent breeding grounds for the anopheles , and voila: malaria reappeared. However: this was a quinine resistent malaria so the world had a new problem. Luckily new drugs were discovered in cardiology: quinolone and quinoleine and lo and behold: they were also effective against malaria. The two I mention are generic names but I'm sure you will find the brands. You should start a preventive treatment about a week before you enter a contaminated area.
