Yes.. infact that buttermilk was probably expired at 3 weeks If it had been opened already.

I often use buttermilk in my baking. I've 'freshened' expired buttermilk by adding milk and shaking. Of course, I do then bake the buttermilk and other ingredients at 350 degrees or so, possibly lessening any potential ill effects.

YES!!!!! It spoils after a few weeks open. Throw that bottle away.
