Liver dysfunction- it can cause bleeding disorders because some of the clotting factors are created by the liver. Platelets travel thru our blood vessels not sticking to everything because they have to be activated by something, like a cut. To put it simply, the cut activates some clotting factors (way too many to name here) and hence platelets attach to form a clot.
Transfusion reaction- likely talking about the mismatch of blood types, like A, B, AB, & O, and (+) &(-) blood types. If you are transfused w/ the wrong blood type, your body reacts to it like an "intruder" and trys to destroy it. Essentially, it means your body is destroying the cells that carry oxygen to your body, and the broken-down cells can also be harmful to you. This happens because the red blood cells have "markers" on the surface that determine if it is (+), (-), A, B, AB, O which is genetically inherited from our parents. Real quick and simple, if your blood type is A+, you can receive A- & A+ blood types. If your blood type is AB+, you can receive A, B, O. If is O-, then only O and has to be (-).
Hemoglobin (Hgb) and hematocrit (Hct) are measurements of the red blood cells. Hematocrit is sort of a calculated hemoglobin, essentially it is a 3-to-1 ratio. For example, if your Hgb is 10, then your Hct is around 30. So if you have a high Hgb, then you will likely have a high Hct.

A cold agglutinins blood test is done to detect conditions that stimulate the body to produce cold agglutinins, or antibodies. Cold agglutinins are usually produced in response to infection. They cause red blood cells to clump together at low temps
Blood received in a transfusion must contain antigens that are the same as the antigens on a person's own red blood cells (compatible blood). If you receive a transfusion that contains antigens different from your own (incompatible blood), the antibodies in your plasma will recognize the transfused (donor) blood as foreign and will attack and destroy the donor red blood cells. This is called a transfusion reaction, and it occurs immediately when incompatible blood is transfused. A transfusion reaction can be mild or cause a serious illness and even death.
Hematocrit (HCT, packed cell volume, PCV). This test measures the amount of space (volume) red blood cells occupy in the blood. The value is given as a percentage of red blood cells in a volume of blood. For example, a hematocrit of 38 means that 38% of the blood's volume is composed of red cells.
Hemoglobin (Hgb). Hemoglobin is the major substance in a red blood cells. It carries oxygen and gives the blood cell its red color. The hemoglobin test measures the amount of hemoglobin in blood and is a good indication of the blood's ability to carry oxygen throughout the body.

A bleeding disorder is a condition that affects the way your blood normally clots this process is also known as coagulation, changing blood from a liquid to a solid.When the liver is damaged, there may be longer than normal time for blood to clot Thus dysfunction of the liver results in deficiency of many clotting proteins leading to bleeding tendency. Another reason is that the liver converts inactive form of some of the clotting factors to active forms with the help of vitamin k.

Your liver manufactures among other things one of the many enzymes needed for the bloodclotting process. Absence of this enzyme will cause bleeding through capillaries.
A transfusion reaction is the forming of antibodies by the mother against the Rh of the baby whilst the fetus is still in utero. These antibodies are passed on to the baby and will probably kill him or her unless a transference transfusion is performed immediately after birth. Presence of those antibodies before the birth is determined by a Coombs test.
