I know that in Maryland you must have a title.
Here, it takes as long as 8 weeks for the DMV to get a replacement title to the registered owner, so your friend may be telling the truth about the delay.
You can insure the car without owning it - that is no problem. I have done that on several occasions. Maybe the seller will let you use his tags if you show him proof of insurance.
But... absent a title, most states will not even issue a paper transport tag...

Do you know for certain that your friend was the only name on the title/registration? That there are no lienholders/legal owners, and no additional registered owners? If your state is running a backlog on their titlework, four weeks would seem about right. You can contact the DMV and ask the status of the title, they can tell you if a request for title has been received. If it turns out that your friend doesn't have legal right to sell the truck, you are out of some cash, and probably won't be friends anymore. You should never buy a car/truck without a title, never. Even the used car dealer has to have the title in his name before he can legally sell, and you can ask to see it unfront, which is what anyone buying a car should do. That title will show you if it is a rebuilt/tottalled car, it will show the true mileage and many other useful items aiding you in your purchase.

A vehicle without a clear title is worthless. Keep after him to follow up and if he doesn't, get your money back. Chances are there is something wrong with the title, it only takes 2 weeks in most cases to get the duplicate.

Don't know the number, but if you don't get it within two weeks, look up the main DMV (off 290 in Houston, TX) and go with him they can issue title at that location.

ask him to write you a bill of sale sometimes if notorized this can serve as permission for you to file for the lost title or go to the title breau ask for a power of attorney for a title and have him sign it this will allow you to file for lost title and then also sign off on the title for him as his power of attorney. if he wont do that for you i suggest you file through the clerk of courts and start a civil lawsuit

your friend should be getting the title soon but if he dont go to dmv with him and tell them whats going on they can help.Or you or he can get it retitled at the dmv but it may have to go thru an inspection (check if its stolen and if its road worthy)by the state.If hes the real owner you should have no problem.
