Right side of face noticeably drooping, but cog. skills seem good.

Physical/Cognitive symptoms and signs will depend on which part of the brain is affected.

Right sided facial droop can certainly be the sign of a stroke, even with good cognitive skills.

However, a right sided facial droop can also be a sign of a disease affecting the right sided facial nerve (see Bell's Palsy). The way to tell the difference is: if they can wrinkle their forehead it's a stroke. If their forehead is smooth when they try to raise their eyebrows, it's a Bell's Palsy type - affecting the Facial nerve. Tricky neurology underlies this.

There was one lady I used to know who couldn't speak, and one side of her body was completely paralyzed. But her mind was sharp as a tack. I'm guessing she had a stroke.
