First step is the hardest. The patient is coming for one of two reasons: Either they have figured out they need help, or the legal system is forcing them to get treatment as part of a plea or conviction.

Drug and alcohol counseling typically comes in many forms. Establishing a rapport with the patient, trying to engage them in conversations and see what their situation is. Homeless and on the street. Trying to get away from a past sin that haunts them. Social use that got out of hand, etc. Once the councelor has determined the best course of action, then it is about directing the patient into a self-diagnostic, and working with them to find out how they can change their behavior. Whether that be changing their social circle, dumping all items, or life symbols, all the while letting go of the old "person" and empowering the new "person" to take strength in their new course, and try to provide options, and support in that direction.

I won't touch on methadone because that gets pretty messy.

Bottom line: You CANNOT help someone (very well) who doesn't want to help themselves.
