Looks like its flea allergy dermatitis. It's quite easy for a vet to identify this as it is very characteristic. If it was just recently treated, it would take time for the skin to recover. Besides, treatment does not stop with your dog, it includes treating the dog's surroundings where the fleas may inhabit.

It could also be chiggers. This could be a problem if you are taking your dog out to poo, and your dog loves rolling on the grass.

It could also be a sign of food allergy. You could shift to a lamb and rice diet, or Hill's z/d (tm) diet, or to Purina's HA (tm) diet and wait for an improvement, afterwhich you could then proceed on identifying source of food allergy.

It could also be allergy to the treatment you're using, it might be too strong. Or it might be predisposing your dog to fungal infestation of its coat.

What age is your lab? It could be just hormonal - a problem with ageing dogs.

Your vet should be able to give a diagnosis before you could go about with any treatment.

Good luck and good health to your lab!

If you can afford IT, take him to another vet.
Just as if you had a question as to a diagnosis given by one physician - and were not completely satisfied with his diagnosis about what was ailing you - would you not trust your presence of mind, to find out, what ALL was responsible for your dis- ease, meaning Lack of comfort as to what had been concluded of you, by just one doctor?
I am not a vet, but WE all make mistakes, mis-judgments
Therefore if you truly feel it in your mind, that the present diagnosis may be incomplete , , then go for a second opinion.
The fact that you have placed this question to US, speaks
of how much you care about your pet.
Therefore, seek Rest of mind, (if it is affordable) through attempting to find what WHOLLY may be troubling your beloved Labrador.
Do you know what a Labrador means in Spanish?
It means what a natural worker will do to help us!
Laborador is the true spelling . . nevertheless, your pet, friend IS there, Has always Been, to help, be of service to YOU! That is his nature!!!
Grace.
