Adapters only really happen to cheep pumps. Modern pumps tend to have two methods. Place the hole over your valve and push down. You then, either turn the bit of the casing that is over the hole until you get a tight seal, before pumping, or you lift a lever to get the seal.
If your not sure, ask at the shop you bought it from.

Basically there are two kinds of tire valve for bicycles;
Shreda and Standard.
The first is a skinny one very common on racers.
The Standar is just like the ones in car tyres, very common in Mountain Bikes. etc.
Some pumps will adapt to both kinds of valves. Most of them are made for one kind of valve, but there are cheap adaptors for them.
If you do not have the box or instructions **** that came with the pump to read the valve size it is made for, then, the best thing is to try if you can connect the pump with the valve, and then see if you can pump air into the tyre.
If not, ask at a bike shop or a knowledgeable friend.

Entirely depends on the type of valve you have on your tyres and the type of pump you have bought. Most good bike pumps nowadays will fit either type of valve. Some may require the changing round of the head of the pump but this will say on the packaging if you need to do this. Some are universal.

As far as I know when you get a bike pump it has a screw on pipe that you attach to the wheel, and there are two types. Best take your bike to a bike shop and ask which pump is the right one for your tyres.

It depends on the pump. Some of the newer floor pumps and some frame pumps will work with either presta or schrader valves. If you're not sure what you have, go to your local bike shop, they will be happy to help you out.

Valves can have various forms, but if your pump nozzle won't fit you will be able to get a different flexible tube for it, or you might be able to exchange the pump. Some bikes have valves like those on car tyres, they need a particular pump nozzle.

almost all bike pumps come with a schrader valve hookup and most tubes use that type of valve so you wont need an adapter. you would need an adapter if you have a Presta valve on your bike. adapters cost about $2

If you got a good brand name pump, it should have both schrader and presta ends on it to pump up either type of tube.
