I have, just takes longer and should be done slowly at first to prevent burning of the outer layer.

If it is the loose stuff in bags then yes you can cook from frozen,

If its in a block then best to defrost because the outside may burn before the middle gets a hint of warmth.

There are no health issues with cooking from frozen so long as it is cooked thoroughly (goes for fresh mince too) and was not going off at the time of freezing

Susan, it is far better to defrost it, even if you use a microwave and keep peeling off the defrosted sections [this will not take long and will keep the juices in the meat] it will also cook faster and easier - cooking a piece of steak is different, thats OK frozen and will taste better on a grill
good luck

Always defrost meat as this will ensure that you do not have any warm rather than hot spots when cooking. The warm spots are where the bacteria will multiply. If you cooked from frozen as suggested on previous answer at a slow rate any bacteria will certainly love those conditions for reproducing.

I have infact cooked mince from frozen. When i was living in london i didnt have a cooker so i used the base from the deep fat fryer! Washed of coarse!! to cook in, as long as it is cooked thourogh and theres no pink bits remaining AND its piping hot it will be ok. Just cook it on a very low heat to make sure the outer bit doenst burn before the rest is cooked>

I would thaw in hot water first for the sake of 10 minutes. I once cooked bacon from frozen (different meat I know) and got severe food poisoning and had to go in Hospital.

Put the meat in a pot of hot water (not boiling...just quite hot) and keep the water hot on the stove till the meat thaws (doesn't take very long), then cook as normal. Might need to reduce cooking time a tad.

I suppose you can by putting it in the oven. Much better to thaw it first so you can add seasoning to spice it up during cooking.
