In general, "by proxy" refers to acting through someone (or something) else. When people vote by proxy, they authorize someone else to vote on their behalf. A medical proxy is someone who makes medical decisions for another person when they're unable to make them by themselves. Even in the computer world, a proxy makes network requests on behalf of other computers that it receives requests from.

A person with Munchhausen's Syndrome makes herself sick to gain attention. A person with Munchhausen's by Proxy makes someone else, usually a child or other dependent family member, sick in order to gain attention as the 'grieving parent/caregiver.'

normal munchausen is when you pretend to be sick for attention. munchausen by proxy is when you (usually a parent) make someone else sick (like poisoning your kid) for attention.
