Traditionally it is the members of the wedding party, which also includes the father of the bride. However, in this day and age with the relaxing of the rigid etiquette rules of Emily Post's reign, I would think that any additional guests could be asked at the grooms and his parents (if they are financing the dinner) discretion.
Personally, I think that if quite a few more than just the bridal party attend, and it goes from a supper to an all night orgy, it should not be referred to as the rehearsal dinner.

I used to work in the wedding industry. You should invite anyone participating in the wedding (wedding party, readers, etc.) and their guests. Also, the parents and grandparents and immediate families of the bride and groom. Some people also choose to invite godparents or out of town guests, but it is nice if you can keep it intimate... the big party will be the wedding reception!
